PURPOSE AND SCOPE

This Agreement (“Agreement”) is made between the Arise Ghana Youth Foundation (AGY) and the Applicant (“You” or “Your”), in relation to your request for financial support under the AGY General Funds Assistance Program.

By proceeding to the application form, you agree to be bound by the following terms and conditions, which govern the use of AGY’s platform for accessing educational, medical, social, or emergency financial support.

1. Purpose of the Program

The AGY General Funds Application platform is designed to:

• Support individuals and organizations in need of assistance for education, medical care, health outreach, event sponsorship, or community aid.
• Enable AGY to assess, verify, and process funding requests in a fair and transparent manner.
• Connect beneficiaries with funding from AGY’s internal budget or external partner contributions.

2. Scope of Assistance

AGY may provide direct financial support, partial sponsorship, medical supplies, educational materials, or referral to partner institutions.

This assistance is not guaranteed and is subject to availability of funds, internal review, and the legitimacy of the request.

 

  TERMS AND RESPONSIBILITIES

3. Applicant Responsibilities

By submitting a funding request, you agree to the following:

• All information provided is true, accurate, and complete.
• You will use the funds strictly for the purpose stated in your application.
• You will submit any requested supporting documents (ID, receipts, letters, etc.) for verification.
• You will not engage in fraudulent activity, duplicate funding applications, or misuse of support received.
• You understand that AGY may request additional follow-up, impact reports, or documentation of outcomes.

If applying on behalf of a group or organization, you affirm that:

• You are authorized to act on behalf of the group.
• You will be accountable for how the funds are used or distributed.

4. Use of Funds

Approved funds may only be used for:

• Tuition, school fees, or exam registration
• Hospital bills, medication, or medical screenings
• Event logistics, transportation, or supplies
• Public outreach or community development costs
• Emergency relief for verifiable crises

AGY reserves the right to recover or terminate aid if funds are used for purposes other than those declared.

5. AGY Rights and Discretion

• AGY may approve or reject your application at its sole discretion.
• AGY may verify all submitted information and conduct independent checks through partners or local reps.
• AGY may publicize anonymized success stories for transparency and donor engagement (with your consent).
• AGY reserves the right to deny future requests if fraud, misuse, or non-cooperation is identified.

 

 CONSENT, PRIVACY, AND DIGITAL ACCEPTANCE

6. Confidentiality and Data Use

• Your personal data and application information will be securely stored and used solely for the purpose of fund allocation, reporting, and compliance.
• AGY will not share your personal information with unrelated third parties without your consent, except when required by law.

7. Non-Entitlement Clause

By submitting this form, you acknowledge that:

• AGY funds are limited and allocated based on need, verification, and impact.
• You are not automatically entitled to funding.
• You may be asked to wait, reapply, or adjust your request based on funding availability.

8. Agreement to Proceed

By clicking “I Agree”:

• You confirm that you understand and accept the terms of this Agreement.
• You agree to cooperate with AGY throughout the review and post-support process.
• You authorize AGY to contact you via email, phone, or WhatsApp regarding your request.

If you do not agree with the terms, please do not proceed with the application.
